Right to Access Your Records
You have the right to inspect and obtain a copy of your Protected Health Information (PHI) maintained by Mino MD. This includes your intake forms, prescriptions, health metrics, check-in data, and messaging history. You may request your records in electronic format.
How to request
Email hello@mino.inc with subject "PHI Access Request." We will respond within 30 days.
Right to Amend Your Records
If you believe your health information is incorrect or incomplete, you may request an amendment. We will respond within 60 days. If we deny your request, we will provide a written explanation and you may submit a statement of disagreement.
Right to an Accounting of Disclosures
You may request a list of disclosures of your PHI made by Mino MD for purposes other than treatment, payment, or healthcare operations. Mino MD maintains comprehensive audit logs of all PHI access, and we can provide an accounting covering the prior 6 years.
Right to Request Restrictions
You may request that we restrict certain uses and disclosures of your PHI. While we are not required to agree to all restrictions, we must comply with any restriction on disclosure to a health plan for payment or healthcare operations purposes when you have paid for the service in full out of pocket.
Right to Confidential Communications
You may request that we communicate with you about health matters through a specific method or at a specific location. For example, you may request that we contact you only by email or only at a certain address.
Right to Be Notified of a Breach
If a breach of your unsecured PHI occurs, we will notify you within 60 days of discovery. The notification will describe the nature of the breach, the types of information involved, steps you should take, and what we are doing to investigate and mitigate harm.
Right to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at hello@mino.inc, or directly with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
Right to Revoke Authorization
If you have provided written authorization for specific uses of your PHI beyond treatment, payment, and operations, you may revoke that authorization at any time in writing. Revocation will not affect any actions already taken in reliance on the authorization.
